Abstract     This artlcle deals with the similarities between periodontal disease (PD) and rheumatoid arthritis (RA). Although the two diseases differ in their etiology, the underlying pathogenetic mechanisms and biochemical processes of tissue destruction are remarkably similar. Recent studies indicate that PD has a much greater prevalence among patients with RA, compared with the general population, but there is as yet no evidence of a causal relationship between RA and PD. Most patients with RA require no special adjustment of the standard dental procedures. However, a detailed history must be obtained, since even a mild form of RA may adversely affect the patient's ability to maintain good oral hygiene. The dentist should provide advice on proper home oral hygiene practices and devices. A review of the patient's medication history allows more accurate differential diagnosis of oral lesions and minimizes complications due to drug interactions or overdosage. Special attention should be devoted to patients with RA who have had a total arthroplasty. According to guidelines of the American Academy of Orthopaedic Surgeons and American Dental Association from 2003, such patients should receive antibiotic prophylaxis before undergoing dental procedures that may induce a transient bacteraemia.
Summary     Članek obravnava podobnosti med revmatoidnim artritisom (RA) in parodontalno boleznijo (PB). Čeprav se etiologiji obeh bolezni razlikujeta, pa so si patogenetski mehanizmi in biokemični procesi razgradnje tkiv podobni. Novejše raziskave tudi kažejo na večjo prevalenco PB pri bolnikih z RA kot pri splošni populaciji, vendar dokazov o vzročni povezanosti med RA in PB zaenkrat še ni. Pri večini bolnikov z RA stomatoloških posegov ni potrebno prilagajati. Potrebna pa je natančna anamneza, saj že blage oblike RA zmanjšajo bolnikovo sposobnost vzdrževanja ustrezne ustne higiene. Pri tem jim moramo pomagati z nasveti o načinu vzdrževanja ustne higiene in uporabi sredstev. Poznavanje zdravil, ki jih bolnik jemlje, nam omogoči boljšo diferencialno diagnozo sprememb ustne sluznice in zmanjša možnost zapletov zaradi medsebojnega učinkovanja ali predoziranja zdravil. Posebno pozornost zahtevajo tisti bolniki z RA, ki imajo vstavljene sklepne endoproteze. Po priporočilu American Academy of Orthopaedlc Surgeons in American Dental Association (2003), potrebujejo ti bolnlkl pred posegi z možno prehodno bakteriemijo zaradi krvavitve antibiotično zaščito.
