Abstract     BACKGROUND: Only 10% of patients with stroke and TIA (transient ischemic attack) are 45 years of age or less. We often find hemostatic changes in the acute phase of ischemic cerebrovascular events (ICE), which indicates an increased tendency to thrombosis in these patients. Data about blood coagulation in the chronic phase of the disease are few and inconsistent. AIM: We studied blood coagulation in young people in the chronic phase after ICE. HYPOTHESIS: We assumed there are coagulation disorders in young patients in the chronic phase after ICE which play an important role in the onset of arterial disease. METHODS: We included 44 consecutive patients (27 women and 17 men), who were referred to our Neurological unit after having suffered from stroke (n=34) or TIA (n=8) at the age of 45 years or less (33,8±6,3 years). Since the ICE 5,3±2,3 years passed. 7 (15,9%) patients had suffered a recurrent ICE. We also included 48 healthy age and sex-matched controls. We examined all the participants and investigated the risk factors for ICE. From venous blood we measured fibrinogen concentration and D-dimer levels and performed coagulation tests for thrombophilia to determine antithrombin, protein C and protein S activity, resistance to activated protein C, homocysteine concentration and the presence of lupus anticoagulants. We also measured total cholesterol, high-density lipoprotein (HDL) and low-density lipoprotein (LDL) cholesterol, triglyceride and blood glucose concentration, haematocrit, and platelet count. The significance of differences between the two groups was analyzed by means of chi2 statistics for categorical variables. Continous variables were compared by means of Student's t-test for normally distributed values, and Mann-Whitney's test for abnormally distributed values. (Abstract truncated at 2000 characters).
