Abstract		Heart failure is a clinical syndrome caused by the inability of heart to provide adequate perfusion to peripheral tissues and organs. In the last decade the number of patients with heart failure has been steadily increasing. The current guidelines represent an efficient and useful tool for treatment of heart failure on primary and secondary level of care. However, despite the significant progress in the treatment of heart failure, a substantial number of patients reach an advanced stage of the disease, which needs to be treated with sophisticated and novel techniques available only in specialized tertiary medical centers. In Ljubljana Medical Center, the currently available modalities for treatment of advanced heart failure include heart transplantation, biventricular pacing and newer pharmacologic approaches. These modalities represent the fundament for introduction of novel therapies including left ventricular assist device support, immunoadsorption, and stem cell transplantation.
Summary		Srčno popuščanje je klinični sindrom, ki nastane zaradi nezmožnosti srca, da zagotovi zadostno prekrvitev perifernih organov in tkiv. Število bolnikov s srčnim popuščanjem z leti narašča tudi na račun staranja prebivalstva. Utečena doktrinarna stališča omogočajo učinkovito zdravljenje začetnih stopenj srčnega popuščanja na primarni in sekundarni ravni. V terciarnih zdravstvenih ustanovah pa svojo pomembno mesto v zdravljenju napredovalega srčnega popuščanja dobivajo novejše metode, ki zahtevajo posebna znanja ter tudi znatnejša finančna sredstva. Na KC v Ljubljani je na voljo presaditev srca, vstavljanje biventrikularnih srčnih spodbujevalcev in zdravljenje z novejšiml zdravili. Ustvarjajo se pogoji zdravljenja napredovalega srčnega popuščanja z mehansko podporo prekatov, zdravljenje z imunoadsorpcijo ter presaditev krvotvornih matičnih celic.
