Abstract     In patients with chronic atrial fibrillation the heart rate during exercise is excessively increased. The aim of the present study was to find out which of the drugs is the best in reducing the heart rate during exercise: whether digoxin or verapamil or sotalol and whether the therapy is necessary at all. In order to find this out, each of the nine patients with chronic atrial fibrillation was treated for a period of 14 days with different drugs. During one 14 days period the patient was treated with digoxin, during the next with verapamil, during the third with sotalol and during another he was not treated with any drug at all. After each 14 days period the patients were exercised on an cycloergometer in accordance with modified Bruce's protocol. During the exercise the heart rate was monitored. We found that in patients which were given for 14 days no medication, the heart rate was excessively increased during exercise. It was increased to such values, as were never reached even in healthy individuals with sinus rhythm at peak exercise. The heart rate during exercise was reduced approximately equally by digoxin, verapamil and sotalol (during the peak exercise for about 15%). On the basis of the above it can be concluded that these drugs are efficient not only during rest but also during exercise.
