Abstract     Background and methods: The patients with non-specific low back pain are treated conservatively and mostly with physical therapy. Literature regarding recent published researches is regularly followed by our team members at Department of Physical and Rehabilitation Medicine of General Hospital Maribor. We are keeping up to date with the latest results of systematic and randomized controlled studies included in Cochrane databases. There is a relatively small number of clinical studies that fulfil the criteria of high methodological quality of evidence based medicine. The effectiveness of therapists' advice to remain active, therapy with non-steroid anti-inflammatory drugs and manipulative therapy has been proved for patients with the acute low back pain. In patients with chronic low back pain the effectiveness of kinesiotherapy, low back school, and bio-psycho-social rehabilitation has been proved. The effectiveness of other drugs, injections, and some of the most frequently used physical therapy modalities has not been confirmed by the evidence based medicine. Conclusion: The prescription of physical therapy for patients with non-specific low back pain remains empirical and is not evidence based. The absence of evidence does not confirm that the therapy is not efficient but rather represents the absence of clinical studies of high methodological quality. There are a few clinical and organization problems that have to be address in the future: insufficient medical education, poorly divided diagnostic therapeutic process between different levels of medical care, and insufficient clinical research work.
