Abstract     After multiple transfusions of foreign red blood cells the patient becomes a chimera, because only ABO and RhD compatibility is considered in blood transfusion and not the compatibility of others blood group systems, such as Kell, Kidd, Duffy, MNS, etc. The serological determination of autologous blood groups in these patients is practically impossible because of mixed red cell populations. In such cases, the genotyping of blood group systems RhCE, Kell, Kidd, Duffy and MNS could be used for determination of autologous blood group types. To reach this task we extended the described ABO and RhD genotyping protocol also to the Kell, Kidd, Duffy and MNS blood group systems. We tested the applicability of this protocol for simultaneous determination of autologous blood groups from the peripheral venous blood of polytransfused patients. The PCR-SSP method was chosen for genotyping. The primers for amplification of different alleles of the Kell, Kidd, Duffy and MNS blood group systems were selected on the basis of the known data on the polimorphisms. The PCR-SSP reactions were validated by using the samples of known phenotypes, obtained from the blood bank registers. Blood samples from 8 patients who received multiple transfusions were genotyped with the new uniform protocol. Genotyping was performed from the samples obtained before tirst transfusion and after 26 multiple-transfusion events. On the average of 6.9 non-white blood cells reduced red blood cells units were administered per single transfusion event. The units of red blood cells were on the average 5.9 days old. The average period of blood sampling after transfusion was 5.3 days. All patient samples and samples of transfused red cell blood units were also paralellely serologically phenotyped for ABO, Rh, Kell, Kidd and Duffy antigens. Additionally, buccal samples from patients were genotyped. The serological results were compared to the results of the genotyping. (Abstract truncated at 2000 characters).
