Abstract     Objective: The aim of our study was to investigate perifollicular blood flow characteristics and vascularization assessed by Doppler technologies and to evaluate the connection between blood flow characteristics and IVF/ICSI outcome in unstimulated cycles. Methods: Quantitative parameters of perifollicular blood flow (PI, RI and PSV) were measured in 210 cycles in 134 patients on the day of HCG administration, the next day (day +1) and prior to follicular aspiration (day +2). Pulsatility index (PI), resistance index (RI) and peak systolic velocity (PSV) were compared between cycles with fertilization (group A) and without fertilization (group B), and between conception cycles (group D) and nonconception cycles (group C). Changes in PI, RI and PSV in consecutive measurements were analyzed in all four groups. Power Doppler (PWD) was used in 141 cycles in 89 patients to assess the percentage of the follicular wall in which blood flow was discovered, and the follicles were classified accordingly into five groups. The frequency distribution of the types of perifollicular vascularity was analysed in the whole studied group. The frequency distribution of the types of perifollicular vascularity was compared between groups A and B, and groups C and D. Results: PI and RI in cycles with fertilization were significantly lower than in cycles without fertilization, but only on day 0. Measurements of PI, RI and PSV on day +1 and day +2 failed to show any significant difference. There was no statistically significant difference in PI, Rf and PSV between conception and nonconception cycles. There was no satistically significant difference in the frequency distribution of different types of perifollicular vascularity between groups A and B or between groups C and D. No pregnancy occurred from follicles with scarce, dotted vascularity. (Abstract truncated at 2000 characters).
