Abstract     Best results for cytology in organized cervical cancer programs can be obtained by raising the sensitivity of the PAP smear test to the calculated levels from epidemiological studies. This increases the sensitivity of the program and the awareness of all partners in the screening program to the negative effects of the screening procedures on healthy women. Organized screening based on the classical Papanicolaou smear was proven to be more cost-effective in the reduction of the incidence of invasive disease and mortality than the opportunistic approach. For a successful program the guidelines, recommendations and standards should be respected at the highest possible level. Organized screening is able to detect irregularities concerning screening procedures and treatment of precancerous lesions. Thus the real cost- effectiveness and weakness of the program could be recognized and measured. A number of elements for quality assurance of the screening program are related to quality assurance as a part of good laboratory practice. In the article some crucial elements of quality control are stressed in order to achieve higher quality of the screening program. There is an urgent need for respect of quality control mechanisms and standards in all screening laboratories.
