Abstract     In recent years EUS was proved to be safe and effective in diagnosing biliopapillary diseases. Early use of EUS can provide the definite diagnosis and guide the therapy, which may include immediate endoscopic treatment in the same procedure. Furthermore, the development of sectorial linear EUS has made it possible to obtain biopsy specimens by EUS-guided fine needle aspiration for cytology. In specialized centers linear EUS with large channel echoendoscopes is now used more and more directly for therapeutic procedures, including drainage of cystic lesions and EUS-guided medicament application. However, the widest application of EUS is in diagnosing and staging of early cancer of Vater's papilla, pancreas and bile ducts. Introducing EUS in diagnostic and therapeutic decision-making algorithms has changed the approach to clinical management in a significant proportion of patients to a less invasive strategy involving less risk and lower cost. The number of applications of endoscopic papillotomy (EPT) is still growing, particularly due to various new techniques. It facilitates access to both bile and pancreatic ducts and allows further endoscopic treatment to achieve normal function, which may be impaired for many different reasons.
