Abstract     Introduction. End-stage renal failure induces a state of immunodeficiency and increases infection risk seen in these patients. Case report. We report a case of a 58-year-old patient with end-stage renal failure treated with hemodyalisis for the last year, who was admitted due to pain in the lower abdomen. During the diagnostic procedure emphysematous gastritis and acute cholecystitis were confrrmed. Acute abdomen developed and the patient underwent emergency laparotomy. During the procedure the necrotic gallbladder was removed. The patient died the following day due to multiorgan failure and septic shock. Conclusions. Emphysematous gastritis is an extremely rare infection of the upper digestive tract. According to the literature this infection is an almost uniformly fatal disease.
