Abstract     Preoperative determination of the course of the primary anal fistula track and its relationship to the external sphincter and puborectal muscles is essential for accurate diagnosis of the type of fistula and for correct choise of therapy. Physical examination and fistulography are of limited value. Conventional transrectal ultrasonography enables better visualization of the fistula and its relationship to the sphincter muscle and the use of injected hydrogen peroxide (H2O2) as a contrast agent during ultrasonography further enhances the imaging of pelvic floor anatomy and relapsing fistulas. The purpose of this study was to show the value of transrectal ultrasonography in the diagnosis of benign anorectal disorders and to present the diagnostic value of individual diagnostic methods (physical examination, fistulography, conventional ultrasound and hydrogen peroxide enhanced ultrasound) in the preoperative diagnosis of perianal fistulas compared to surgical examination under anesthesia. The objective was also to demonstrate a quantitative and qualitative contribution of hydrogen peroxide enhanced ultrasound examination to diagnostic accuracy and therapeutic decision-making. The study included 40 patients with perianal fistulas diagnosed at outpatient clinics. Preoperatively, they all had fistulography, conventional ultrasound examination and hydrogen peroxide enhanced ultrasound examination. They subsequently underwent surgery at the Department of General and Abdominal Surgery of the Celje General Hospital. Patients with perianal fistulas were classified into 5 diagnostic groups.
