Abstract     Objective: Acute appendicitis is the commonest acute abdominal surgical condition. Diagnosis of acute appendicitis in children is especially difficult, because of atypical clinical findings. The consequences are both a high rate of perforation and a high rate of negative appendectomy. History and physical examination remain the golden standard for diagnosis of acute appendicitis. Diagnostic accuracy is partially improved with use of routine laboratory tests such as: white blood cell count, differential count and C-reactive protein (CRP). The objective of this study was to evaluate whether the two newer markers of inflammation: interleukin-6 (IL-6) and lipopolysaccharide binding protein (LBP), produced also by enterocytes, have better diagnostic accuracy for acute appendicitis in children than currently routinely used laboratory tests: white blood cell count, differential count and CRP. We also wanted to compare diagnostic accuracy of laboratory parameters with diagnostic accuracy of ultrasound examination and specific clinical signs. Two hypotheses were established: 1. Serum IL-6 and LBP concentration have better diagnostic accuracy for acute appendicitis in children than white blood cell count, differential count and serum CRP concentration. 2. Diagnostic accuracy of IL-6 and LBP is similar to diagnostic accuracy of specific clinical signs and ultrasound examination. Patients and methods: Forty-nine children who underwent surgery for acute appendicitis and 33 children who were admitted because of suspected acute appendicitis, which was later ruled out, were included into the study.
