Abstract     Traurnatic brain injury is the leading cause of death in multiple trauma patients. Two main aims of management of head-injured patients are: to provide the best conditions for recovery from brain damage already sustained and to prevent or treat complications leading to secondary brain damage. In preforming advanced prehospital life support of head injured patients, the traditianal ABCDE scheme of assessment and intervention is followed. In the evolving situation, careful and continual reevaluation is essential to detect any signs of deterioration. Prevention and treatment of hypoxemia, hypercapnia, aspiration, securing the airway with cervical protection, ensuring adequate ventilation and prevention of hypoperfusion are the main methods of treatment. Careful quality control of the performance of prehospital ALS is needed, paying particular attention to delays in the field and to the effectiveness of the procedures undertaken.
