Abstract     In patients with brain injury, special attention is given to measures for decreasing secondary brain impairments (apart form the support to respiratory function and the stabilization of blood circulation, there are various pharmacological or physical methods for the reduction of brain metabolism). A lot of attention has been given to the discovery of specific mediators of the secondary brain impairment and to research of various possibilities for impeding their development or activity. In the case of head injuries, the most frequent cause of death remains the uncontrollable intracranial hypertension. It used to be treated with therapeutic measures for decreasing the intracranial pressure (ICP). The current therapeutic approach is directed towards the optimization of the cerebral perfusion pressure (CPP = mean arterial pressure - intracranial pressure), which should be over 60 mmHg. Due to this approach, the treatment has changed in many ways. The optimal treatment of brain injury requires understanding of pathophysiological processes taking place after brain injury.
