Abstract     Transcranial Doppler sonography is a non-invasive, available and reproducible diagnostic test detecting blood flow velocity in cerebral arteries. This is usualy kept constant by the autoregulation but hypo- or hyperperfusion, vasospasm and embolism can cause cerebral damage. Monitoring coronary artery bypass grafting and carotid thrombendarterectomy using transcranial Doppler sonography enables us to recognize impared perfusion and cerebral microembolism on time. Thus it can help us to reduce the incidence of postoperative neurologic sequellae. The latest recommendations on use of transcranial Doppler sonography monitoring during coronary artery bypass grafting and carotid thrombendarterectomy are rated as probably useful. To get a more established rating, additional data from large scale prospective studies are needed.
