Abstract     The aetiology of chronic inflammatory bowel disease is unknown. Treatment is, as a rule, conservative (medications). Surgical treatment is reserved for complications of the disease. Patients with Crohn's disease cannot be cured by surgery, so in Crohn's disease surgical treatment is organ-preserving surgery - limited resections of severely affected bowel or/and stricturoplasty. Ulcerative colitis can be cured by radical surgery, but at a very high price, as all the large bowel and rectum must be resected. Indications for such a radical operation, restorative coloproctectomy, which is the procedure of choice, should be considered very carefully.
