Abstract     The prognosis of breast cancer is mostly dependent on its ability to metastasize. Different proteolytic factors, including cathepsins D and L, are involved in tumour invasion and metastasizing. Despite numerous studies, the prognostic value of cathepsin D in breast cancer remains controversal. The data on the prognostic value of cathepsin L are still very scarce. The primary aims of our study were to assess the prognostic significance of cathepsins D and L as well as of their combinations with other proteolytic factors in breast cancer patients. The secondary aims were to analyse the role of both cathepsins and their combinations in the identitrcation of patients with a very low risk of disease recurrence who could be spared adjuvant systemic therapy, and to evaluate the prognostic impact of both cathepsins and their combinations in comparison to that of established prognostic factors. A total of 715 patients with primary operable breast cancer were entered into the study. Of them, 615 patients with primary operable breast cancer were entered into the study. Of them, 615 patients were treated at the Institute of Oncology between 1996 and 2000, and the remaining 100 underwent a curative surgical treatment at the General Hospital Celje in the same period. Immediately after surgery, tumour cytosols and triton extracts were prepared for biochemical analysis. The antigen content of cathepsin D in tumour cytosols was determined by the commercially available immunoradiometric assay test. The levels of cathepsin L were determined in triton extracts of tumours by ELISA test. (Abstract truncated at 2000 characters)
