Abstract     Background. Evaluation of health needs and the outcoming development of health care systems are two processes, which have for a long time been developed independently one from another. Definition of needs was the basic process out of which stemmed other decisions. The role of individual partners was changing through history. Methods. The development of methods has come a long way from the early 20th century, when the need for health care was evaluated principally based on demography. Today, methods are directed at processes and assessment of factors both inside health care as well as more broadly, from the public health point of view. Apart from demography, we need data about the subjective perception of health status and the societal burden of disease, represented both by single diseases as well as by the total national pathology. Discussion. Relationships between the patient-doctor axis and the environment, which has as its key partners payer(s), politics and citizens, the latter being either employers or employees, are the key element for a complex assessment of population health needs. Medicocentric and clinical models find themselves in a crisis, precipitated by the fact that their development was based on high growth rates of health expenditures. On the other hand, health needs are nowadays seen in a broader context with an increasing role of the public health approach. (Abstract truncated at 2000 characters)
