Abstract     Infections represent an important complication of heart transplantation. Epidemiology of infections has changed in recent years. The most common are bacterial infections followed by cytomegalovirus infection and various other infections. Aspergillosis is the most common fungal infection in heart transplant recipients and carries a great mortality. legionella and Toxoplasma cause other specific infections in these patients. Pre and posttransplant strategies for patient management are discussed in the article as well. Since 1990 46 heart transplantations were performed in Slovenia. In the early postoperative period and in the first year after the transplantation 13 patients died and infection was the major cause of death in 3 (23 %) patients. Two of them died due to invasive disseminated aspergillosis.
