Abstract     When medical therapy in acute heart failure doesn't prove successful, noninvasive ventilation should be considered before intubation. Mostly we choose between providing continuous positive airway pressure alone or adding a pressure support for patient's spontaneous breaths. Over 50% of intubations may be avoided in patients without contraindications for noninvasive ventilation. The method proves particularly successful in hypercapnia. Monitoring is not demanding in noninvasive ventilation, but it must be attentive. Failure of recognizing deterioration may lead to delay in intubation. In such case, noninvasive ventilation brings more harm than benefit.
