Abstract     Spontaneous hemopneumothorax is a potentially fatal condition associated with 0.5% of all spontaneous pneumothoraces. The usual source of bleeding is a torn apical vascular adhesion band. In the past 7 years, 378 cases of spontaneous pneumothorax were treated at our institution. Four of them, all male, aged 18-33 years, presented with spontaneous hemopneumothorax and a blood loss of 1500-3500ml or 25-60% of estimated blood volume. All were successfully cured, three by urgent thoractomy and one by pleural drainage alone. We describe a case of spontaneous hematopneumothorax.
