Abstract     Naloxone should be used very carefully in heroin poisoning with concomitant methadone (25%) and ecstasy/cocaine poisoning (15%), because in such cases naloxone application results in agitation and seizures more frequently. Therefore naloxone dosing should be very cautiously, the starting naloxone dose should be only 0.2 mg. Agitation and seizures after naloxone application can be treated with diazepam. Hospital observation for 3 hours is necessary for recurrent hypoventilation and pulmonary edema.
