Abstract     In introduction the history of all past radiological accidents with the cumulative number of victims is shown. The biological effects, principles of radiation injury and dosimetric principles are presented. The biological effects of radiation are deterministic and undeterministic. The deterministic ones are dose and dose-rate dependent. The subclinical manifestation is blood picture change; the clinical manifestation is dose dependent expression of radiation sickness. The undeterministic effects are malignant alterations and genetic damages. The connection with the dose is statistical. Radiation sickness starts with initial phase with nausea and vomiting, which is followed by latent phase with blood picture changes and ends in manifest phase. This phase has three manifestations (which one will be expressed depends on received dose): the hematological, the gastroenterological or the cerebral. The exit is recovering or death. The first aid measures are measures in the place of accident, measures after admitting the pacient to hospital and the medical support. The first aid measures are ABC and prevention of further irradiation. After admitting to hospital the main symptoms will be restlessness, nausea and vomiting. The continuing symptomatic will be in dependance of accepted dose the cerebral, gastroenterological or hematological.
