Abstract     Patients with painfull and stiff neck and fever can represent a big diagnostic problem. There can be a lot of such clinical pictures. It can be a result of trauma and patient should be considered to have a cervical spine fracture or ligamentous injury until proven otherwise. In the absence of trauma, meningitis, encephalitis and subarachnoid hemorrhage must be considered. We also have to think about local spinal processes and inflamation processes in internal structures of the neck. Neuroleptic agents and antiemetics can cause extrapyramidal side effects (dystonic reaction with neck pain and stiffnes, fever and protrusion of the tongue). In our case we had a young male patient, with extrapyramidal symptomps after consumation of antiemetic thiethylperasin.
