Abstract     Pulmonary embolism is defined as a partial or a complete occlusion of the pulmonary artery by thrombi, which are usually caused by deep venous thrombosis. Clinical picture depends on the number and the size of emboli and underlying lung and heart disease. Massive pulmonary embolisms is usually manifested as an unexpected dispnea, obtuse chest pain and syncope. Patients tend to be hypotensive, tahycardic, tahypnoic and to show signs of right-sided heart failure. According to medical doctrine, massive pulmonary embolism is treated with thrombolysis. lt induces faster dissolving of thrombs and therefore faster normalization of pulmonary flow. Thrombolysis is contraindicated in peri-operative period. We are presenting an example of a thirty-six years old patient, where the massive pulmonary embolisms occured in the post-operative period. Because of the serious hemodynamic instability, we decided to use the thrombolysis treatment in spite to contraindication.
