Abstract     Chronic heart failure (CHF) is patholophysiological state, in which heart is not able to produce enough output to meet the needs of peripheral tissue while operating at a normal or enhanced left ventricular filling pressure. CHF is seen as a syndrome accompanied by simptoms and signs of pulmonal and peripheral congestion. Ischaemic heart disease is the most frequent cause of CHF in developed countries. In most cases left ventricular (LV) systolic function is impaired. The assessment of LV function by conventional echocardiography is essential in determining patients with CHF. Recent epidemiological studies have established that as many as half of all patients with CHF have preserved LV systolic function (normal or slightly impaired ejection fraction > 45-50%). They tend to be older, more frequently women, and to have history of hypertension. In these patients, diastolic dysfunction is implicated as primary cause of CHF. Most recent studies have shown that systollc and diastolic dysfunction could not be separated into two pathophysiological entities, moreover, they coexists in most patients with CHF.
