Abstract     In contrast to the evidence-based treatment of systolic heart failure where a lot of controlled clinical trials are available to guide the management, the treatment of diastolic heart failure remains largely empirical since trial data are limited The goals of treatment of diastolic heart failure are following: first, to relieve symptoms mainly by decreasing puImonary venous pressure at rest and during exercise; second, to treat the precipitating factors leading to acute decompensation such are uncontrolled arterial hypertension, myocardial ishemia and atrial fibrillation with tachyarhythmia, third, the long-term optimal control of pathologic process that causes diastalic heart failure, for example coronary artery disease, arterial hypertension, aortic stenosis, etc. With some exceptions, many drugs that are used to treat diastolic heart failure are in fact the same as those used treat systolic heart failure. However, the rationale for their use, the pathophysiological process that is being altered by the drug, and the dosing regimen may be entirely different depending on whether the patient has systolic or diastolic heart failure.
