Abstract     The postoperative recurrence of varicose veins is diagnosed in 40% of the cases and 20% of all varicose vein surgeries are repeated treatments. The postoperative anatomic conditions are significantly altered and require a detailed diagnosis prior to the second procedure. The article offers the guidelines for performing the colour coded ultrasonography on such patients and the criteria, which confirm a recurrent venous insufficiency. In the groin area the recurrence is best defined by employing the Edinburgh classificatioon, it is also necessary to rule out the non-saphenofemoral reflux as in 16.3% of the cases the connection through pudendal and gluteal veins has been noted. In 22% the saphenopopliteal junction remains unchanged and in 15.7% there is a possibility of a connection of the lesser saphenous vein with the greater saphenous vein through the Giacomini vein. The ultrasonography also entails an estimate of the incompetence in perforators and in deep venous system. The preoperative ultrasonographic vein mapping will likely help avoid any further recurrence and the consequent advancement of the clinical presentation of chronic venous insufficiency.
