Abstract     Primary aldosteronism (PA) affects 4.5% of patients with hypertension. Patients with hypertension and hypokalemia, resistant hypertension or adrenal incidentaloma should undergo screening for PA with a plasma aldosterone concentration to plasma renin activity ratio. A high ratio warrants confirmatory testing for PA. For those patients with PA who want to pursue a surgical cure, the accurate distinction between autonomous aldosterone secretion from one or both adrenal glands is the next step. Laparoscopic adrenalectomy follows when aldosterone secretion is unilateral, otherwise treatment is medical with aldosterone antagonists.
