Title     Vloga lokalnih kortikosteroidov kot možnih ekcematogenov in njihov vpliv na Demodex folliculorum pri bolnicah s perioralnim dermatitisom

Abstract     Perioral dermatitis (PD) is a chronic dermatosis of the perioral region occuring mainly in young and middle-aged women. Its aetiology and pathogenesis are unclear. It may be trigged or aggravated by different external factors, including topical steroids, which are often prescribed for its treatment. The mechanism underlying the adverse effects of topical steroids in PD is not adequately understood and has not been investigated. The main purpose of our study was to examine the role of topical steroids in PD from two points of view: their potential eczematogenous effect, and their influence on a specific component of facial skin microflora - the mite Demodex folliculorum (DF). Besides, we wanted to assess the frequency of contact allergy to ointments used for facial skin care and/or their ingredients in our patients. We also wished to investigate the prevalence and density of DF in patients with PD not treated with topical steroids and in healthy female subjects of different age. A standardized skin surface biopsy of the chin was performed in 82 female patients with PD and in 70 age-matched control female subjects. The study group included 53 patients who had been treated with topical steroids and 29 patients who had not received such treatment. Patch tests with contact allergens of the standard test series, ointments used for facial skin care, and two markers for contact allergy to topical steroids (tixocortol pivalate and butesonide) were performed additionally in 37 patients and 35 control subjects. Summary     Perioralni dermatitis (PD) je pogosta kožna bolezen obraza nepojasnjene etiologije in patogeneze. Pojavlja se predvsem pri ženskah v mlajšem in srednjem življenjskem obdobju. Med številnimi zunanjimi dejavniki ga lahko sprožijo ali poslabšajo tudi t.i. lokalni kortikosteroidi (LK), ki jih za zdravljenje te bolezni pogosto predpispejo. Mehanizmi neželenega delovanja LK pri PD niso pojasnjeni in doslej niso bili raziskani. Osrednji namen naše raziskave je bil ugotovili vlogo LK pri tej bolezni z dveh vidikov, njihove potencialne ekcematogenosti in vpliva na specifični del kožne mikroflore obraza, gostoto pršice Demodex folliculorum (DF). Hkrati smo želeli oceniti pogostnost kontaktne alergijske preobčutljivosti za kozmetične pripravke za nego kože obraza. Ugotoviti smo želeli tudi prevalenco in gostoto DF pri bolnicah s PD, ki se niso zdravile z LK in pri zdravih preiskovankah v različnih starostih. Pri 82 bolnicah s PD in 70 kontrolnih zdravih preiskovankah primerljive starosti smo opravili površinsko biopsijo kože na bradi in ocenili prevalenco in gostoto DF. V raziskovani skupini se je 53 bolnic predhodno zdravilo z različnimi preparati LK, 29 bolnic pa LK ni uporabljalo. Pri 37 bolnicah s PD in 35 zdravih preiskovankah smo dodatno opravili epikutano testiranje na standardne alergene, negovalne pripravke za nego kože obraza in markerja za kontaktno preobčutljivost za LK- toksikortol pivalat in butezonid. Pri bolnicah. ki so se zdravile z LK. smo opravili epikutano testiranje tudi z vzorci teh preparatov. (Izvleček skrajšan na 2000 znakov)
