Abstract     Palmoplantar pustulosis (PPP) is a chronic, disabling and recalcitrant pustular dermatosis affecting palms and soles. Its onset is strongly linked to cigarette smoking and once established it tends to progress gradually over years or decades. It seems that there may be a relationship between PPP, autoimmune diseases and alterations of the thyroid function. The relationship of PPP to palmoplantar pustular psoriasis is not clear cut. Histologycally it closely resembles psoriasis. Newer investigations of the PSOR 1 locus in the major histocompatibility complex region suggest that PPP could be a distinct disorder. The treatment is unsatisfactory and many different therapeutic manoeuvres have been advocated. In randomized controlled trials the best results were achieved with systemic retinoid therapy on its own or in conjunction with PUVA therapy . Over a 5 year period (2000-2004) we studied 38 patients (87% female and 13% male) diagnosed with PPP. - 92 % were actual or previous smokers ; 8 had never smoked. - 11% had a family history of psoriasis. The mean age at the onset was 45 years . The mean duration of the disease - from the first appearance was 4,5 years. In our patients a thyroid disease was not registred.
