Abstract     Very preterm neonates, those born near the second and third trimester, suffer from different forms of brain damage rarely observed in term neonates. These disorders can have early and long consequences and are often associated with cerebral paralysis and epilepsy while 50 % of these children suffer from cognitive, behavioral and learning disorders at school age. At our neonatal ICU we use US examination of the brain to diagnose hypoxic-ischemic alterations of cerebral matter. The first US examination of the brain is carried out between postnatal days 1-7, another between postnataldays 5-15 and one between days 1660. The MR image or the newer diffusion- based MR image provides a better diagnostic approach for assessment of diffuse brain tissue injury in preterm neonates.
