Abstract		The changes in surgical principles of gastric cancer treatment developed in the last three decades are described. Gradual rise of 5-year survival rate in gastric cancer patients on national level and on connectives 5-year series of own department is presented. Actual surgical treatment principles are described: extent of resection in all three dimensions (on the organ itself, on neighbourning structures and organs or their parts), extent of lymphadenectomy and methods of reconstruction. New less invasive methods of treatment are mentioned too.
