Abstract     Background. Multivisceral resections (MVS) in advanced gastric cancer patients in stage III and IV are questionable due to severe postoperative complications and poor prognosis. Patients and methods. Prospectively collected MVC patients' data in the 20-year period are analysed. In this period 1552 gastric cancer patients were resected, 79 of them with MVR. Results. 22 out of 798 MVR were subtotal gastric resections and 57 total gastrectomies. Postoperative mortality rate (30-day): subtotal resection 9,1% (2/22), total gastrectomies 14,0% (8/57). 5-years survival rate in all MVR patients was 13,4%, in RO resected ones 16,9%. In subtotal MVR 23,5%, in total MVR gastrectomis 13,9%; in resected patients with only one additional organ 18,2% (44/79), with two organs 12,5% (8/79). Conclusion. In selected cases of stage III and IV gastric cancer MVR gives gastric cancer patients the chance of longer survival with relatively good life quality.
