Abstract     Extension of lymphadenectomy in gastric cancer is controversial, and preoperative diagnosis of lymph node metastases is difficult. Therefore, knowledgebased systems such as the Maruyama computer program are being developed. The aim of this study was to determine the feasibility of Maruyama computer program in patients with gastric cancer for the assessment of regional lymph node status and preoperative planning. We analysed 1 15 patients with gastric cancer treated in Clinical department for abdominal surgery, UMC Ljubljana from 1.1.1993 to 31 .12.1998. All patients underwent standard gastrectomy with systematic D2 lymphadenectomy. After resection pathologic N status was compared with the prediction of the computer program. Using the Maruyama computer model, a sensitivity of 88% for the correct prediction of the lymph node involvement was associated with a specificity of 90% and a positive predictive value of 92%. Results of the Maruyama computer model and the sentinel lymph node biopsy can be compared for their value to predict the nodal status and lead to stage-adapted surgery.
