Title     Nekateri klinični in biološki napovedni dejavniki pri bolnikih z neoperabilnim karcinomom ustnega žrela zdravljenih s sočasno radiokemoterapijo

Abstract     In our study, the influence of some clinical characteristics of patients and tumors as well as tumor biomarkers on the treatment outcome in patients treated for advanced inoperable oropharyngeal carcinoma by concomitant radiochemotherapy with Mitomycin C and Bleomycin, in the years 1990-97 was investigated. We asumed, that knowing the predictive value of these factors would contribute to a better customizing of treatment for patients. Among clinical characteristics supposed to influence the outcome of the disease, we studied performance status, age, stage of the disease, and among the biomarker proteins the influence of p21, p27, p53, EFGFR, cyclin D1, CD31 and Ki67 which were determined in the tumour by immunohistochemistry. Patients were followed up from 46 to 146 months (median 85 months). At the time of the evaluation of the results, 25 out of 95 patients (26%) were still alive. Forty two patients (44%) died of oropharyngeal carcinoma, 16 (17%) of a new primary tumor with no evidence of the progression of the oropharyngeal carcinoma, and 12 (13%) of the causes unrelated to the cancer. At the fifth year of follow-up, the probability of local and regional control, diseases-free survival, and of overall survival was 55%, 51%, and 32%, respectively. After the completed treatment, distant metastases developed in 14 (15%) of patients, whereas 18 new primary tumors were detected in 17 (18%) patients. (Abstract truncated at 2000 characters)
