Abstract     Background. Every interruption of the skin's continuity, surgical wounds included, results in scar formation. Scars - hypertrophic and keloid in particutar - represent an esthetic disturbance. Quite frequently the esthetic disturbance is associated with functional impairment of varying extent. We attempt to reduce scars and their sequelae in various ways, often using surgical methods. Plastic surgery has developed numerous methods to reduce and correct skin scars, and to eliminate the functional impairment caused by the scars. Methods. The author presents a method of solving functional impairment caused by hypertrophic and keloid scars using free fullthickness grafts. He presents several types of free skin grafts as well as the principles of free skin transplantation, with special stress on free transplantation of full-thickness grafts. Conclusions. The application of free full-thickness grafts is one of the simplest methods for the solution of functional impairment caused by hypertrophic and keloid scars. Accurate diagnosis is imperative for satisfactory functional results while the esthetic results are somewhat poorer compared with other scar-reducing surgical techniques. The method is most frequently used in post-burn contractures, particularly on the hands.
