Abstract     Restoration of normal gastrointestinal function is a key aspect of postoperative care and is promoted by the use of enhanced recovery programs. The malnourished patients are at high risk of postoperative complications and nutritional support is the integral part of perioperative therapy.The proper nutritional status should be maintained or optimized before before the operative therapy. With this approach the operative metabolic stress is minimized and the likehood for the development of pooperative complications is lover. Postgastrectomy syndrom encompasses nutritional intolerances and deficiences. Frequent intolerances include dumping syndrome (early and late), fat maldigestion, lactose and casein intolerance and malabsorbtion syndrome. Combinations of these are most likely responsible for acute postoperative weight loss, the most frequent complication of gastrectomized patients. Micronutrient deficiencies devolop months or years after gastric resections and can results in deleterious clinical consequencies. Nutritional support is a part of the patient metabolic care. It should be considered on an individual basis for all patients undergoing major surgery and also in the planing of nutrition when the postgastrectomy syndrom has developed.
