Abstract     Although declining in incidence in western countries during the last twenty years, gastric cancer remains an important cause of cancer-related death throughout the world. Prognosis of gastric cancer is poor, as it is usually diagnosed late. More attention should be paid to the early detection to improve the effectiveness of treatment and thus survival of patients who develop gastric cancer. Surgery remains the cornerstone of the management. The curative treatment of gastric cancer is now becoming the subject of a multidisciplinary approach. Actual surgical treatment principles are desribed: extent of resection in all three dimensions (on the organ itself, on neighbourning structures and organs or their parts), extent of lymphadenectomy and methods of reconctruction. New less invasive methods of treatment are mentioned too.
