Abstract     Background. Maxillary sinusitis is important because of its frequency, a lot of days of restricted activity and increased antimicrobial prescriptions, what influence on bacterial resistance to antimicrobials in population. Methods. The contemporary guidelines for treatment of acute sinusitis are presented according the adequate literature. At the same time the chronic sinusitis is decribed mostly from the view of odontogenic causes and oromaxillofacial procedures according the modern literature as clinical experiences. Results. The acute maxillary sinusitis is overtreated by antimicrobials according the modern literature as clinical experiences. Results. The acute maxillary sinusitis is overtreated by antimicrobials according the modern knowledge about its pathogenesis, also the guidelines about the antimicrobials of first and second choise are changed. The chronic sinusitis beside the infection is the most frequently conditioned by systemic and local states, between them the most frequently are the anatomic changes of upper respiratory part and inflammatory illnesses of upper premolars and molars. Conclusions. Adequate treatment of the acute and chronic maxillary sinusitis can have important influence on the quality of treatment itself and the bacterial resistance on antimicrobials. For the prophylaction of the maxillary sinusitis we can successfully act against the factors that may increase risk of sinusitis.
