Abstract     Background. Closed reduction and external fixation is a new and promising method of treatment whit which we try to diminish limited mobility frequently seen after the treatment of closed unstable proximal phalangeal fractures. We have compared our results of treatment of fractures of the metacarpal bones and the proximal phalanges of digits with a.) functional immobilization, b.) open reduction and internal fixation and c.) closed reduction and external fixation. Methods. We have prospectively analyzed clinical resufts of all stable or after reduction stabilized fractures treated with functional immobilization. Results obtained in years 1990-1992 with open reduction and internal fixations were retrospectively analyzed. Clinical results of first hundred patients treated with closed reduction and external fixation were prospectively analyzed. In this study good clinical outcome means that total active movement (TAM) of alI joints of the finger is equal or greater than 230 degrees and excellent means full and unrestricted mobility. Results. Between 26 patients treated with functional immobilization good clinical outcome was found in 4 of them and excellent in 20. Similar results were obtained with closed reduction and external fixation (good 20/94, excellent 63/94). With open reduction and internal fixation only 20/43 of good or better results were obtained (good 10/43, excellent 10/43). Conclusions. For closed and after reduction stabilized fractures in acceptable position functional immobilization is recommended. Similar results can be achieved with closed reduction and external fixation of unstable fractures.
