Abstract     The authors present a short review of modern trends in oncologic reconstructive surgery with its final aims-quality of life improvement, good function and pleasing appearance. Listed are differences in wound healing after radio and chemotherapy. Stressed are the advantages of immediate reconstruction in a single operation at the time of tumor ablation by free tissue transfer and microvascular anastomosis, in particular reliability and speed which guarantee beginning of adjuvant therapy 14 days after tumor ablation and reconstruction. Listed are preoperative, pre-anastomotic, inter-anastomic, post-anastomic and post-operative factors influencing free flap survival or loss.
