Abstract     Pleural effusion occurs either as a result of pleural disease or due to disorders of other organs. With diagnostic procedures the classification of pleural effusion into a transudate or an exudate should be made and the underlying disorder should be identified. The first-line invasive diagnostic procedure is thoracentesis, which can also be used as a therapeutic modality It is a safe procedure and can be repeated if necessary Pneumothorax is the most common complication. The underlying disorder causing pleural effusion should always be treated. The next therapeutic procedure is tube thoracostomy. A chest tube is inserted percutaneously into the pleural space, either under ultrasound or X-ray guidance. Indications include hemothorax, large pneumothorax and empyema. Patients with malignant pleural effusion can be treated with repeated thoracentesis. For patients who are in a good general condition, the best treatment is pleurodesis, which is used to produce serositis and subsequent fibrosis.
