Abstract     Two patients treated for septic shock in the Maribor Teaching Hospital Medical Intensive Care Unit are presented as an illustration of the clinical picture of septic shock in the elderly. The first patient is an elderly woman with acute pyelonephritis that presented itself with a mild clinical picture at onset but progressed rapidly to septic shock needing intensive treatment and hemodynamic support. Treatment was further complicated by the presence of previously unknown congestive heart failure. The patient needed temporary inotropic support and mechanical ventilation. With intensive care and timely antibiotic therapy she made a full recovery The second patient is an elderly woman institutionalized in a mental institution due to oligophrenia. She was admitted to hospital with an atypical dinical picture - impaired level of consciousness and electrolytic dysbalance without clear evidence of infection. During hospitalization we discovered acute cholecystitis and respiratory infection leading to sepsis and septic shock. After immediate antibiotic therapy and intensive care she returned to the institution fully recovered.
