Abstract     According to the published data and latest guidelines, therapeutic hypothermia must be used in all unconscious adults with spontaneous circulation after out-of-hospital cardiac arrest due to VF or pulseless VT. Main contraindication is serious bleeding. Simple and cheap method of cold infusion, combined with external surface cooling is comparable to today's invasive devices with respect to efficacy as well as safety. Cooling must be initiated as soon as possible after the return of spontaneous circulation - preferably prehospitaly. Cause of arrest must be treated at the same time (e.g. patients with STEMI must be brought to cardiac catheterization laboratory without delay). Therapeutic hypothermia might also benefit other unconscious non-traumatic cardiac arrest patients after ROSC (unshockable rhythms or in-hospital arrests) and is therefore reasonable in those circumstances.
