Abstract     Acute hemiparesis is not so often seen in childhood as it is in adulthood. We can assume many differerent causes according to differential diagnostic. Early diagnosis is important beacuse of specific therapy. The paper presents the case of acute hemiparesis in 15,5 years old previously healthy boy. The primary angiitis of central nervous system (PACNS) was confirmed. Disease is quite rare in childhood. Diagnosis of PACNS is based on clinical features, laboratory results of cerebrospinal fuid and angiographic features of central nervous system. PACNS may follow progressive or nonprogresive course. Confirmation of the diagnosis is crucial for early treatment with corticosteroids. progresive PACNS other immunosupressive drugs are used beside corticosteroid and antithrombotic treatment.
