Abstract     Epistaxis or nasal bleeding is a frequent complaint. It is usually a simptom of pathology in the nasal cavity, the nasopharynx or the paranasal sinusis. In the literature we have not encountered a case where epistaxis ocurred due to gastrointestinal bleeding. We are reporting a case of a patient with epistaxis as a simptom of an arterial vessel haemorrhage from a penetrant gastric ulcer. Rapid fall in haemoglobin levels, pronunced leukocitosis and most of all deterioration of patients condition with the progression to haemorrhagic shock which all sugested an active internal bleeding urged us to perform endoscopy of the upper GI tract despite a history of epistaxis. The patient was succesfully treated with endoscopic haemostasis, intensive management of haemorrhagic shock and a proton pump inhibitor.
