Abstract     In the article, the authors present an older patient who was admitted to our department due to exacerbation of asthma and right sided heart failure. Despite appropriate treatment patient's condition deteriorated and resulted in respiratory failure. Regarding clinical signs we suspected pneumothorax, which we did not prove with chest imaging. As pleural effusion was also excluded wetook into consideration aspiration of a foreign body (food particle). Respiratory failure was caused by mucus in airways, which produced lung atelectasis. Mucus was abundant because of asthma exacerbation and very viscous most propably because of (over)treatment of right side heart failure with diuretics. After therapeutic bronchoscopy patient's condition improved and he was soon released from hospital.
