Abstract     Following is the description of a cave accident, in which a 35-year-old speleologist, who at a depth of 120m sustained a compound fracture of the femur, was involved. The intervention was carried out by the Speleological Rescue Service (SRS). Exceptionally grave circumstances demanded medical measures to be constantly re-adapted to answer the situation: the dressing of wounds could not be sterile, the position of the injured man during the transport was not horizontal all the time, parenteral application of medicaments included very little liquid, the doctor could exercise only sporadical control of the injured man, a particular system was applied to keep the injured man warm.
