Abstract     The main purpose of chest tube insertion is to drain the pleural cavity of any air and/or fluid (blood, pus, transudate, etc.). The doctor on emergency call often deals with seriously injured patients with chest trauma and hemothorax or pneumothorax that require immediate emergency room thoracostomy. Therefore a question is raised: would it be aproppriate for him to perform an urgent chest tube or needle thoracostomy in a prehospital setting? Main principles of relieving the pleural cavity are described and good and bad points of prehospital thoracostomy discussed.
